Effective January 1, 2012, Facilities and physicians will utilize (8) new codes for the application of Apligraf.

- Current application codes 15340, 15341, G0440, G0441 are being deleted effective 1/1/12
- New application codes will be based on wound size/location and will carry a 0 day global billing period
- These new application codes will be used by all providers in all places of service
